ADHD Evaluation Process
01
Your First Appointment- Getting to Know You
In your intake session, we'll take the time to understand you. We'll review your medical and mental health history, review your childhood, explore your daily habits, and discuss your goals. The first session is about building trust and creating a plan together. Medication will not be prescribed during your first evaluation. You will be provided with an 89-item questionnaire and 2 observation forms that must be completed before your next appointment. If you have a previous ADHD diagnosis, please upload your medical records to the patient portal. If you don't have them, we will have you sign a release of information form and request them on your behalf.

Follow Up- Understanding Your Challenges
During your second session, we will go over your answers to the questionnaire to get a better understanding of the challenges you're experiencing. We will also go over the observation forms. If medication is medically necessary, we'll explore medication options and review lifestyle changes that may help. You'll also receive educational resources to empower you with knowledge and practical tools.

Second Follow Up- Assessing Progress
During your third session, if medication was prescribed, we'll discuss how it's working for you and make adjustments if necessary. If not, we'll explore medication options. We'll also discuss any progress you've made with lifestyle habits. Together, we'll introduce a simple Cognitive Behavioral Therapy (CBT) exercise to work on before your next session.

Ongoing Care-Growing with Each Session
Each session builds on the last. We'll provide a new CBT exercise to help you develop skills for managing challenges. At every step, we'll review your progress, adjust the plan if needed, and celebrate your heard work.
*Please note, each treatment plan is individualized, and some plans may deviate from the plan listed above. We no longer provide controlled substances for treatment of ADHD.
